Things you will need to register:
1. USA Hockey online registration confirmation: You will not be able to register without the USA Hockey Confirmation page. go to the following link https://www.usahockeyregistration.com/login_input.action
2. Check/Cash for registration/calendars
3. Copy of Certified Birth Certificate (If first year player)
4. Completed Consent to Treat Form
Important Information About Registration
All players must be registered through USA Hockey first. You will need to pay the USA Hockey fee of $50.00 online with a credit card before you register with DAHA. EXCEPTION: MITES WITH BIRTH YEAR of 2007 TO PRESENT STILL NEED TO REGISTER WITH USA HOCKEY BUT DO NOT GET CHARGED THE $50.00 FEE.
